For the purpose of the Personal Data Protection Act, I consent to the disclosure by CJC of any personal information that i have provided to CJC ("the Personal Information") to other agencies or individuals for the purpose stated below
- To contact me for any other purpose related to online service and/or assistance that will be/has been provided, or on matter to which i have an ongoing relationship with CJC; and
- To be used as aggregate data for statistical purposes
I am aware that the CJC will keep my personal information for 12 months
